Up One Level Icon Disappearing

After integration with iRIMS we periodically lose our "Up One Level" icon. We also do not have the ability to move up the navigation tree with the pulldown menu. This makes navigation very difficult. This seems to be random as we cannot re-create it and appears to act differently on different clients.Has anyone seen this problem before. Any comments would be appreciated.
