Unknown appearing in Add New Item dropdown

Infosys_Infosys_(infosysuser1_-_(deleted))

Whenever I create a new module, instead of the LLNode fname, a unknown appears in the Add New Item dropdown list. I have checked and rechecked my module and everything seems to be alright. Can anyone suggest what the problem could be.This is very urgent. Pls help if you have any info.Regards,Amit Jhunjhunwala
