Report for Folder Permissions on Specific Userid

I'm trying to set up a report for Folder Permissions on specific Userids whereas the person running the report can enter in the userid of their choice.Any ideas how to set this up using SQL ? I have the schema book, but not sure what field names I'm looking for.
