Use builder to customize template attribute search

We have developed a form template that we would like to use in a search - searching more than one field at a time. AS a first choice, we would like to the attributes from our form template to the Advanced Search page as search criteria. Or if the search of attributes cannot be added to the existing search, we would like to develop a search similar to advanced search but only searching fields in our form template. Please let me know the best method to do so.Thank you.
