How to Add Attachments to a SQL based Form?

Hi Friends, I have a SQL based Form , wherein users fill values, which are stored in Custom table and then retrieved by Functional administrators through Livereports. I had a Qns..Is it possible for the users to attach a document to the SQL based Form ?if yes, can someone plz give me the hint how to do the same? RegardsMA Naim
