This error could be anything. Mandatory attribute not filled, not enough free space on storage, and so on. You could launch the builder and trace your LAPI call on the Livelink server itself. Or if you don't have the builder, set debug to 11 in opentext.ini and try to find the reason in logs. Also try to add a document via the web interface (browser) to see if you can do it the standard way.
