Report on all workflows run for a particuular document

We use workflows for Document Management and we have both signing workflows and non-signing workflows. I would like to be able to run a query showing me the Workflow History for a specific document.1) which version was run through workflow2) name of workflow map3) initiator4) users performing user steps5) dispositions of those user steps6) date initiated and7) date completedCan anyone help me idetify the way forward with this?
