7.6 install - HELP (again)

Chaining does not always work
I have an initial action that chains to another action. Sometimes it works and sometimes it doesn't. The error in the error log just says "Failed to chain action:" and then gives the action name. Modeling activities conducted by different roles
What is the best way to model tasks/activities that are done at the same time by different roles? I have used the cloning and alias feature, but this has created confusion among our end users. Anyone have any other suggestions?
